We Accept Most Commercial Insurance Plans and Medicaid

In-Network Commercial Plans: BCBS, Premera Blue Cross, Anthem, Aetna, Cigna, First Choice, United Healthcare (Commercial), Regence Blue Cross, Healthcare Management Administrators, Uniform Medical Plan, Asuris NW, Lifewise, Ambetter Coordinated Care, Boeing, Kaiser (Access PPO/Options Federal/Summit PPO/Elect PPO plans only), Tricare

In-Network Medicaid Plans: WA HCA - DSHS (Traditional Medicaid/Apple Health), Molina Healthcare, Amerigroup/Wellpoint, Coordinated Care of Washington, United Healthcare Community, Medicare (for patients 19-21 years only), DMAP - Oregon Medicaid

We are currently out of network with CHPW (Community Health Plan of Washington) Medicaid plan.***

Unable to accept patients with Kaiser HMO plans at this time. 

All prospective CHAMPVA patients are required to call the clinic to discuss prior to scheduling. CHAMPVA patients are only able to be accepted into the practice if there is a separate primary coverage that is active and maintained. We cannot bill CHAMPVA as a primary insurance payer. 

Please have your insurance card with you at the time of your visit. Co-pays are due at time of visit, and credit/debit cards only are accepted in the clinic.

WE BELIEVE THAT QUALITY HEALTHCARE SHOULD BE ACCESSIBLE TO ALL. Self-pay packages are available for children needing care who are not insured. 

- Well checks $200: includes all screenings, exam, in-house lab tests, vaccines, and form completion if needed

- Sick visits $150: includes exam and any in-house testing or treatments needed

- Sports Physicals $85: includes exam and form completion

- Form only $25: no exam, only form completion required for school or camp

- Vaccines only $23.44 per vaccine administration
